Share and embed forms with Peak Forms

A form only works once it reaches the right people. With Peak Forms you publish once and use the same link everywhere: in emails, in chat, on posters or embedded right in your website.

Start for free. The link works as soon as you publish.

One form, many ways to reach the people who answer it.

Ways to share

One published form, four ways out

Every way leads to the same public address, so answers always land in the same form, wherever someone starts.

Live demo

This is what an embedded form looks like on a website

Below, a real Peak Forms experience runs in an embedded frame, just as it would on your site. Switch between desktop and phone and try it.

In three steps

Embed a form in your website

  1. Publish

Check your form and put it live. Only then do the link and the code work.

  2. Copy the code

Open “Share” and copy the embed code for your website.

  3. Paste

Put it into your page's HTML block. The height adjusts automatically.

Peak Forms creates the code for your published form. This is what it looks like:

Embed code (example)
<iframe src="https://app.peak-atlas.com/f/YOUR-FORM-ID?embed=1" title="Contact request" loading="lazy" style="width:100%;min-height:640px;border:0" data-peak-form="YOUR-FORM-ID"></iframe>
<script>addEventListener("message",function(e){if(e.data&&e.data.peakForm==="YOUR-FORM-ID"){document.querySelectorAll('[data-peak-form="YOUR-FORM-ID"]').forEach(function(f){f.style.height=e.data.height+"px"})}})</script>

Peak Forms replaces “YOUR-FORM-ID” with your form's public address automatically. The internal form id stays private.

Platforms

Where the embed code works

The code is plain HTML. It works wherever you may add your own HTML. There are no dedicated extensions for individual platforms yet.

  • Content management systems

For example WordPress or Webflow: add an HTML or code block and paste the snippet into it.

  • Website builders

For example Wix or Squarespace: use the embed-code element. Some builders only allow scripts on paid plans.

  • Online shops

For example Shopify: embed the form on a page through a custom HTML section, for requests or feedback.

  • Your own HTML

Into any page you build yourself: place the code where you want it. Without the script the form still works at a fixed minimum height.

Shared safely

Change things without breaking shared links

The link stays the same while you rework your form. You decide what is live.

  • Same address Link and embed always point to the version that is currently live.
  • Fixed versions Published versions never change. Every answer remembers which version it belongs to.
  • Back with one click Put an earlier version live again or continue it as a draft.

How do I embed a form in my website?

Publish your form, open “Share” in Peak Forms and copy the embed code. Paste it into an HTML block on your page. The form appears there and adjusts its height itself.

Do respondents need an account?

No. Anyone who opens your link or sees the embedded form can answer right away.

Does the link change when I edit the form?

No. You edit a draft and publish a new version. Link and embed stay the same and show the version that is live.

Can I see where answers come from?

Yes, with UTM parameters on the form address, for example for newsletters, ads or social media.

Are there pop-ups or plugins for my platform?

Not yet. Pop-up, side panel and platform extensions are in preparation. Today you embed with a snippet that works on any common platform with an HTML block.
