Use case · Customer feedback
Find out who recommends you and who doesn't.
One NPS question, a targeted follow-up on criticism and a note to you when someone is unhappy. A good feedback form needs nothing more.
The e-mail is optional. Whoever wants to stay anonymous stays anonymous.
The feedback template
Three questions, two results.
At 0 to 6 the follow-up appears and you get notified. Everyone else gets a thank-you.
- Question 1
- How likely are you to recommend us? (0–10)
- At 0–6
- “What would we have to change to make it a 10?”
- Question 3
- What did you like best?
- Result
- Criticism → review + notification
Template from Peak Forms
Questions from the German template, translated for this page. You adapt them freely.
Why it works
Short for customers, clear for you.
One number
The NPS question adds to the referral dimension. You see the trend per form.
Follow-up only on criticism
Happy customers are done after two questions. Critics explain what's missing.
Criticism first
Critical responses land in your Dialogs as “review” and trigger a notification.
How to start
Live in ten minutes.
Pick the template
Open “Collect feedback and NPS” in the editor.
Adapt the questions
Insert the name of your offer, adjust the tone, keep the follow-up.
Send the link
Put the link in your wrap-up mail or share it on site with a QR code.
Frequently asked questions
How is NPS calculated?
Share of 9–10 minus share of 0–6. You find the free NPS calculator under Tools.
Can I ask CSAT instead of NPS?
Yes. Swap the NPS question for a 1 to 5 rating. You adapt the logic to the new scale.
Are the responses anonymous?
As long as nobody gives an e-mail, yes. With an e-mail you can follow up personally.
